The Genentech BioOncology Co-pay Card

If you have commercial insurance, such as one through your employer or Healhcare.gov, Genentech offers the BioOncology Co-pay Card that may be able to help you with the out-of-pocket costs for GAZYVA.

The Genentech BioOncology Co-pay Card is available only for commercially insured patients. Patients using Medicare, Medicaid or any other government funded program to pay for their medications are not eligible. It requires a valid, on-label prescription and cannot be combined with any other rebate/coupon, free trial or similar offer for the specified prescription. It is available only for a patient (or their guardian) who is 18 years or older. It is not valid for medications the patient receives for free or that are eligible to be reimbursed by private insurance plans or other health care or pharmaceutical assistance programs that reimburse the patient in whole or in part for the medication. It is valid only for Genentech products in the United States and Puerto Rico. The Genentech BioOncology Co-pay Card is not health insurance or a benefit plan. Please see full terms and conditions at https://www.copayassistancenow.com/#/terms-conditions.

Independent Co-pay Assistance Foundations

If you have public or commercial insurance, GAZYVA Access Solutions can refer you to an independent copay assistance foundation* that may be able to help with out-of-pocket costs for GAZYVA.

*Genentech does not influence or control the operations or eligibility criteria of any independent co-pay assistance foundation and cannot guarantee co-pay assistance after a referral from Genentech Access Solutions. The foundations to which we refer patients are not exhaustive or indicative of Genentech’s endorsement or financial support. There may be other foundations to support the patient’s disease state.

The Genentech® Access to Care Foundation

If you don’t have health insurance, or if you can’t afford your Genentech medicine, you could receive GAZYVA free of charge. You need to meet specific criteria to be eligible.
